Patient Centered Care

Introduction

Patient-centered care is defined as an important aspect of healthcare in which there is emphasis on the partnership which is developed in between the nurses and the patients (Delaney, 2018). This essay deals with reflecting upon an incident using Gibbs reflective cycle which focuses on patient-centered care. 

Gibbs reflective cycle

Description

During my placement in the tertiary hospital, Singapore, a 50 year old patient was encountered who arrived at the hospital with the chief complaint of chest pain and dyspnea. He complained that he was suffering from chest pain since 5-6 hours. At the time when he was admitted in the hospital’s emergency ward cardiac arrest occurred and his condition worsened. He was immediately shifted to the ICU for extracorporeal membrane oxygenation (ECMO). Resuscitation was to be provided to treat his condition and improve his health. The patient had undergone the surgery. My duty was to provide assistance to him and fulfill his needs. On the second day after the surgery I forgot to provide him one of his medicine dosage which led to worsening of his condition. 

Feelings

I felt very sad that I was unable to assist the doctor and due to this the condition of the patient worsened. I also had a lack of confidence in providing appropriate care to the patient and was not able to fulfill the needs of the patient.

Evaluation

This incidence gave me a negative experience and failed to provide appropriate care to the patient. The bad thing about this experience was that I did not fulfill the need and patient-centered care was not provided to the patient (Moon et al., 2017). The good thing about this experience was that I realized my mistake and learnt that patient-centered care is important in nursing practice.

Analysis

From this situation I analyzed that the condition of the patent worsened because I failed to fulfill the needs of the patient and assist the doctor for fulfilling the patient needs.  I also analyzed that in this incident failure of provision of patient-centered care to the patient (Lloyd, Elkins & Innes, 2018). I failed to provide him appropriate medications as prescribed by the doctor and this worsened the condition of the patient. Natan & Hochman (2017) stated that provision of patient-centered care is an important aspect of nursing and must be followed by all the nursing professionals.

Conclusion

I learnt that if the patient is not provided with appropriate care then his condition may worsen. Further, this incident gave me knowledge that it is necessary to give patient-centered care by fulfilling all the needs of the patient. In this incident I failed to provide medications to the patient which worsened the patient condition. This represented lack of patient-centered care in my practice.

Action plan

In my future practice I will make sure that I focus on providing patient-centered care to the patient under my care. I will also make sure that I fulfill all the needs and requirements of the patient to provide him comfort and avoid worsening of the patient condition (Hower, Vennedey & Hillen, 2019).

Conclusion

It can be concluded that provision of patient-centered care is an important aspect of nursing practice. It can also be concluded that appropriate care must be provided to the patient and the needs must be fulfilled appropriately.
